- Child's Birth Certificate
- Child's Vaccination Chart
- Aadhar Card of child and both parents
- Fitness Certificate of the child by a registered Doctor
- Transfer Certificate(if applicable)
- Passport Size photos of child (6 copies each measuring 3.5*3.5)
- Passport size photos of both parents one each
